Social Care Network
Need help finding resources?
Social Care Network & Navigation
At Herkimer County HealthNet, we know that health is about more than doctor’s visits. It’s also about having the resources and support you need in everyday life.
Our Social Care Network & Navigation program helps eligible Herkimer County residents identify their social care needs and connect with community resources and services. Through screenings, referrals, and personalized navigation, we help individuals access support related to food, housing, transportation, and other needs that affect their health and well-being.
Our trained Social Care Navigators provide free, confidential screenings to identify needs in areas such as:
- Food access and nutrition support
- Safe, stable housing
- Transportation to work, school, or medical care
- Childcare and family resources
- Utilities assistance
- Mental health and wellness services
- Community programs and benefits
How it Works:
Screening
We start with a quick, friendly conversation, in person or over the phone, to understand your needs.
Navigation
We connect you directly to local and regional resources that can help, walking you through each step.
Follow-Up
We check in to make sure you were able to access the services and get the support you need.
