Herkimer County HealthNet is a grant funded non-profit that focuses on raising awareness of healthcare needs in rural Herkimer County.  We advocate for systemic changes and work to reduce health disparities in our rural area.

What we do

The Mission of Herkimer County HealthNet is to improve the health and well-being of individuals who live, work, play, and learn in Herkimer County. 

Through collaboration, we regularly identify health and human service needs in Herkimer County and facilitate programming to address those needs.

Social Care Network

At Herkimer County HealthNet, we understand that good health goes beyond doctor’s visits — it’s also about having access to the everyday resources and support you need. Through our Social Care Network & Navigation program, we help individuals and families connect with the services that make life easier and healthier. Need help finding resources? We can link you to food, housing, transportation, and other essential services — all at no cost to you. Click the link below to learn more.
