Herkimer County HealthNet is a grant funded non-profit that focuses on raising awareness of healthcare needs in rural Herkimer County. We advocate for systemic changes and work to reduce health disparities in our rural area.
What we do
Since 1989, this is the work we’ve been doing at Herkimer County HealthNet. Our mission is to improve the health and well-being of the people who live, work, play, and learn in Herkimer County, regardless of age, income, ability, or circumstance.
Through collaboration, we identify health and human service needs across the county and build the programs and partnerships that address them.
With over 21% of residents over 65, this initiative works to reduce barriers and expand opportunities so older adults can age with dignity, independence, and connection.
A five-year, $1.6 million NYS-funded initiative bringing schools, municipalities, and community partners together to expand access to healthy food and physical activity for kids countywide.
As Herkimer County’s NYS-designated Rural Health Network, we work to improve access to care and strengthen coordination among providers in rural communities.
At Herkimer County HealthNet, we understand that good health goes beyond doctor’s visits — it’s also about having access to the everyday resources and support you need.
Through our Social Care Network & Navigation program, we help individuals and families connect with the services that make life easier and healthier.
