Who We Are
Since 1989, Herkimer County HealthNet has been bringing people, organizations, and resources together to improve the health and well-being of our community.
As Herkimer County’s designated Rural Health Network, we serve as both a connector and a convener, helping residents navigate available resources while bringing community partners together to identify needs, address gaps in services, and develop collaborative solutions to local health challenges.
From connecting families with essential resources to supporting healthy aging, promoting wellness, and strengthening community partnerships, our work takes place behind the scenes and directly with residents, to reach them at every stage of life.

Good health is about more than what happens in a doctor's office. It's also about whether someone has enough food, safe housing, reliable transportation, access to information, opportunities to stay active, and support when life changes.
With over 21% of residents over 65, this initiative works to reduce barriers and expand opportunities so older adults can age with dignity, independence, and connection.
A five-year, $1.6 million NYS-funded initiative bringing schools, municipalities, and community partners together to expand access to healthy food and physical activity for kids countywide.
As Herkimer County’s NYS-designated Rural Health Network, we work to improve access to care and strengthen coordination among providers in rural communities.
At Herkimer County HealthNet, we understand that good health goes beyond doctor’s visits — it’s also about having access to the everyday resources and support you need.
Through our Social Care Network & Navigation program, we help individuals and families connect with the services that make life easier and healthier.
